Concussion Awareness and Management Protocol and Acknowledgement Form: Each student athlete and parent/guardian needs to familiarize themselves with the District's Concussion Protocol. The District will supply each student athlete with a copy as a reference and guide to understand the process followed by the school'd athletic trainer if a brain injury is to occur. The Acknowledgement form assures the District that the Concussion Policy has been read to its fullest and understood by both student athlete and parent/guardian. This form needs to be singed and dated by both the student athlete and parent/guardian and turned into the athletic director prior to or on the first mandatory practice date.
Sudden Cardiac Arrest Information- Parent and Student Form.pdf
This form assures the District that both the student athlete and parent/guardian are aware of the possibility of SCA and has been educated on signs/symptoms, risks associated with SCA , and removal from play procedures determined by the state medical board. This form needs to be signed and dated by both the student athlete and parent/guardian and turned into the athletic director prior to or on the first mandated practice date.
